In-person offices in Provo, Lehi, and Draper, plus statewide telehealth. Accepting many commercial plans, Medicaid, and Medicare where available. Utah also offers PRIME for Life©, a highly effective education program, available in person or online, for teenagers and adults. Offices in Springfield, Salem, Corvallis, Portland, and Silverton — with telehealth across Oregon. Trauma-informed care with insurance and OHP pathways.
